ACTIVITIES COVERED  I understand that I/my child may voluntarily participate in activities sponsored, organized, hosted, or permitted by United Christian Church ("UCC"),  whether on UCC property or at another location. Covered activities include sports (including basketball, volleyball, soccer and badminton), games,  practices, tournaments, AWANA, children/youth/young adult ministries, VBS, camps, retreats, outings, fellowship events, recreational activities,  playgrounds, inflatables, water activities, physical exercise, and other UCC activities or events.  

ACKNOWLEDGMENT AND ASSUMPTION OF RISK  I understand that sports, games, recreational activities, events, and presence on church property may involve inherent and other risks, including falls,  collisions, equipment-related injuries, cuts, bruises, strains, fractures, concussion, illness, dehydration, heat-related illness, allergic reactions, property  damage, serious bodily injury, disability, and, in rare circumstances, death. I knowingly and voluntarily accept and assume these risks, whether known  or unknown, foreseeable or unforeseeable, to the extent permitted by law.  

RELEASE AND WAIVER OF LIABILITY  TO THE FULLEST EXTENT PERMITTED BY TEXAS LAW, I RELEASE, WAIVE, AND DISCHARGE UNITED CHRISTIAN CHURCH AND ITS  PASTORS, ELDERS, DEACONS, DIRECTORS, OFFICERS, EMPLOYEES, MINISTRY LEADERS, COACHES, TEACHERS, VOLUNTEERS,  AGENTS, REPRESENTATIVES, AND MEMBERS (THE "RELEASED PARTIES") FROM CLAIMS OR LIABILITY ARISING FROM MY/MY  CHILD'S PARTICIPATION IN UCC ACTIVITIES OR PRESENCE AT UCC EVENTS OR PROPERTY, INCLUDING CLAIMS ARISING FROM  THE ORDINARY NEGLIGENCE OF A RELEASED PARTY, TO THE EXTENT SUCH CLAIMS MAY LAWFULLY BE RELEASED UNDER  TEXAS LAW. THIS RELEASE DOES NOT APPLY TO CLAIMS THAT CANNOT LAWFULLY BE WAIVED OR RELEASED.  

PARTICIPANT RESPONSIBILITIES  I/my child will follow reasonable safety instructions and facility rules, use equipment as directed, refrain from intentionally dangerous or inappropriate  conduct, report injuries or unsafe conditions, and stop participating when directed by an authorized UCC leader. UCC may restrict participation when  reasonably necessary for safety or conduct.

PERSONAL PROPERTY  I understand that UCC is not responsible for loss, theft, or damage to personal belongings brought to UCC property, activities, or events, except to the  extent liability cannot legally be waived.  

TERM OF WAIVER  This Waiver and Release is intended to apply on an ongoing basis to all activities, programs, ministries, sports, games, practices, tournaments,  events, trips, outings, recreational activities, and other functions sponsored, organized, hosted, or permitted by United Christian Church, whether  conducted on United Christian Church property or at another location. This Waiver and Release will remain in effect for all future covered activities and  events in which I/my child participates unless and until I revoke it by providing written notice to United Christian Church, or to the extent otherwise  limited or prohibited by applicable law. I understand that United Christian Church may require a new or additional waiver for particular activities,  programs, or events.  

SEVERABILITY  If any provision of this agreement is determined to be invalid or unenforceable, the remaining provisions shall remain effective to the fullest extent  permitted by law.  

ACKNOWLEDGMENT  I acknowledge that I have carefully read this entire document; understand the risks associated with participation; have had an opportunity to ask  questions; understand that this document releases and waives certain legal rights; and sign voluntarily. If signing for a minor, I represent that I am the  participant's parent or legal guardian and have authority to sign this document.
